What Your Adrenal Glands Actually Do (and why they matter more than you think)

You’re sleeping eight hours, but you're still exhausted. You rely on caffeine to get moving, crash in the afternoon, and somehow feel wired when it's finally time for bed. If that sounds familiar, your body may be telling you it's struggling to adapt to chronic stress.

Meet one of your major stress-handling body systems, the adrenals glands - the “underdog” of the hormonal system.

It seems like the thyroid gets all the attention. Meanwhile, the adrenal glands are the overlooked members of the endocrine system, quietly working behind the scenes every single day. Your adrenal glands are about the size of a walnut, yet they influence nearly every system in your body. They help regulate your energy, blood sugar, immune function, inflammation, stress response, and even hormone production. Understanding what these tiny glands do can help explain why chronic stress doesn't just make you feel overwhelmed - it can affect how you sleep, think, eat, and feel every day.

The adrenal glands produce several important hormones, including cortisol, adrenaline, and DHEA. Together, these hormones help your body respond to stress, maintain energy, regulate blood sugar, support immune function, and communicate with other hormone-producing glands. DHEA also serves as a building block for sex hormones like estrogen, progesterone, and testosterone, making healthy adrenal function especially important as we move through perimenopause and after menopause, when the adrenal glands play a greater role in supporting hormone balance. We'll explore each of these roles in more detail in future articles, but first let's look at how your brain and adrenal glands work together.

So how do the adrenal glands know when it's time to release cortisol? That's where an incredible communication system between your brain and adrenal glands comes in.

Let’s take a look at the HPA (hypothalamic-pituitary-adrenal) axis.

The HPA axis regulates our stress response.

Think of the hypothalamus as the executive in charge that runs the show. When a task (responding to a stressor) needs to be carried out, this gland in-charge sends a chemical message, signaling that a task needs to be completed. This signal is sent to the pituitary gland.

The pituitary gland acts as the executive’s assistant (2nd in charge) that then passes the signal on to the adrenals, telling the adrenals to release cortisol (our stress handling hormone).

As cortisol levels rise in response to stress, your body shifts into survival mode and the body’s normal duties become reprioritized. Cortisol helps prepare your body to respond to stress by increasing the availability of glucose for energy, directing more blood flow to your muscles, and promoting alertness. Now the body focuses on the stress that is in front of it and everything else is secondary. Important duties like immune system regulation, digestion and hormone production become secondary until the body has a handle on the stressor. Once the stressor has passed, cortisol levels begin to fall. This signals the brain that the threat has ended, allowing the body to return to its normal balance (homeostasis).

This is a great adaptive process when we encounter normal stress. But what happens when we experience long-term, chronic stress? The important duties mentioned above (like immune system regulation, blood sugar balance, digestion and hormone production) can become suppressed for long periods of time. This is why chronic stress can feel like it affects everything. One day you're exhausted. Another day your digestion is off. Your sleep isn't restful. Your hormones feel out of balance. These symptoms may seem unrelated, but they're often connected through the body's stress response.

One of the fascinating things about the stress response is that your brain responds to perceived threats much the same way it responds to real physical danger. Whether you're slamming on the brakes to avoid a car accident or lying awake replaying tomorrow's presentation in your mind, your body activates many of the same physiological pathways. To your brain, both situations signal that it's time to prepare for survival.

Our stress response was designed to protect us, not to stay switched on all day long. The problem isn't that your body responds to stress - it's that modern life often gives it very little opportunity to fully recover. Work deadlines, family responsibilities, financial pressures, poor sleep, and even our own thoughts can keep sending signals that we're under threat. Over time, that constant state of "high alert" can begin to influence everything from blood sugar and digestion to sleep, immune function, and hormone balance.

Your body doesn't work in isolated systems. Stress affects blood sugar. Blood sugar affects hormones. Hormones affect sleep. Sleep affects inflammation. Everything is connected. When we begin to understand those connections, we can stop chasing individual symptoms and start supporting the body as a whole.
